EPM's Customer Satisfaction Survey

Please fill out the survey and you will be entered in a drawing for a 15% discount certificate for your next order. The drawing for all of those who completed the form will be once every 3 months. The drawings will be held March 30th, June 30th, Sept. 30th and Dec 30th of 2012.

The winner will be notified by mail with the discount certificate and the result will be published on our website and Facebook page, the Monday following the drawing.
